We Accept

Patient’s Auto Personal Injury Policy (PIP)
- Patient MUST initiate claim with insurance
- Info to get from Patient:
  - Name and Phone # of Adjuster
  - Claim #
Patients Private Health Insurance:
- When patient is NOT as fault
- Patients MUST have a Subrogation Rider (Form from ins. Saying who
  is at fault)
- Medicare and Medicaid policy will not have a Subrogation Policy
Private Pay
- Same as Self Pay just a different form for patient to sign
- Can NOT file as Private pay and on private insurance
Payment from Patients Attorney
- Info to get from patient:
  - Name and Phone # of Attorney
- MUST be paid from attorney prior to visits

3rd Party Claims( Auto Insurance of person at fault)
